A Freedom of Information Request by Janet Finch-Saunders, Member of the Welsh Parliament for Aberconwy, has seen Betsi Cadwaladr University Health Board (BCUHB) disclose that between November 2021 and November 2022:
“there have been 14 discharge-planning failures recorded via the Health Board’s on-line incident reporting Datix system whereby the patient did not appear to have a planned care package in place prior to the patients discharge. This is according to the reported incident at the time and specific details recorded of the incident.”
Commenting on the situation, Janet said:
“Whilst the Health Board has warned that the numbers provided should be considered as indicative only, what they prove for certain is that there is a pattern of patients being discharged without care packages in place even where they should be.
“My family have experienced the seriousness of the situation first hand, with a 98-year-old relative discharged from hospital and left sat in a chair at home, hardly able to move.
“But for the family’s intervention, my relative would have been stuck in a chair until carers arrived the next day. I am devastated to think that other vulnerable individuals are being sent home without appropriate support.
“There is clear evidence that the breakdown in communication between health and social care in North Wales is posing a serious threat to vulnerable residents’ wellbeing, so I would like to see the Welsh Government take legislative action to ensure that nobody else is left in such a terrible position again”.

Mae Cais Rhyddid Gwybodaeth gan Janet Finch-Saunders, Aelod o’r Senedd dros Aberconwy, wedi gweld Bwrdd Iechyd Prifysgol Betsi Cadwaladr (BIPBC) yn datgelu rhwng Tachwedd 2021 a Thachwedd 2022:
"cofnodwyd 14 achos o fethiant cynllunio cyn rhyddhau cleifion drwy system cofnodi achosion ar-lein Datix y Bwrdd Iechyd lle nad oedd yn ymddangos bod pecyn gofal wedi'i gynllunio ar gyfer y claf cyn ei ryddhau. Mae hyn yn ôl yr achos a gofnodwyd ar y pryd a manylion penodol yr achos.”
Yn rhoi sylwadau ar y sefyllfa, dywedodd Janet:
"Tra bod y Bwrdd Iechyd wedi rhybuddio mai niferoedd dangosol yw’r rhain, maen nhw’n profi bod patrwm lle mae cleifion yn cael eu rhyddhau heb becynnau gofal yn eu lle hyd yn oed lle y dylen nhw fod.
"Mae fy nheulu wedi profi difrifoldeb y sefyllfa ein hunain, gyda pherthynas 98 oed wedi'i ryddhau o'r ysbyty a'i adael yn eistedd mewn cadair gartref, prin yn gallu symud.
"Heb ymyrraeth y teulu, byddai fy mherthynas wedi bod yn sownd mewn cadair nes i ofalwyr gyrraedd y diwrnod wedyn. Mae'n ddistryw i mi feddwl bod unigolion bregus eraill yn cael eu hanfon adref heb gefnogaeth briodol.
"Mae tystiolaeth glir fod y methiant cyfathrebu rhwng iechyd a gofal cymdeithasol yng ngogledd Cymru yn peri bygythiad difrifol i les trigolion bregus, felly hoffwn weld Llywodraeth Cymru yn cymryd camau deddfwriaethol er mwyn sicrhau nad oes neb arall yn cael ei adael mewn sefyllfa mor ofnadwy eto".
